I've experienced such an issue on Gentoo Linux with xf86-video-ati-6.9.x and Xorg server >= 1.4 (IIRC) and on Ubuntu 8.10. When X starts on my laptop *and* an external monitor is plugged in (either VGA or DVI), both screens start all black, no image is displayed. Switching to a console doesn't even work. I have to power off and restart my laptop. The machine reacts to keyboard input (at least Caps lock and Num lock) but I can't see anything else on any monitor. OTOH starting running xrandr *after* plugging an external monitor does produce the expected results. It means I must plug my external monitor *after* X starts. That issue never happened with radeonhd driver.
