Fixing the DVI/HDMI issue will also fix LVDS.  The issue is that xrandr is
treating DVI and HDMI separately even though they are driven by the same
physical encoder.  Since there are only 2 crtcs, randr grabs them for DVI and
HDMI which leaves nothing for LVDS.
