Ah, yes indeed -- the "Apply to all pages" option is only effective for plain (non-PDF) page choices. The issue is: if you are editing a file that has 5 pages of PDF + 2 pages of plain white paper, and you suddenly load as background a 2-page PDF file, what exactly does it mean to want to apply this to all pages? (I'm sure there could be a sensible answer, but at first it wasn't obvious to me). Even if you load a PNG or JPEG image, it's not clear to me that you *really* wanted it to go onto all pages -- while if you select to say you want plain paper on all pages, that's a pretty clear instruction. Perhaps the name of the option should be changed to make it clearer what it does. Or perhaps I am not imagining quite the right scenarios. (I understand you have PDF files with your favorite paper style templates and want to use them on all pages, which is a very compelling scenario; the scenario where you have a long PDF file being annotated + inserted a few pages of blank paper or other images in the middle to put extra annotation is also quite compelling and perhaps more common, though I never checked). This may need more thought. In any case, you're right: better a mostly functioning thing and no crash (even if "apply all pages" is not functional in this use case) than a crash.
