I also had a similar problem with 4.1.0 and a Voodoo 3 but for me I just had to leave 
it for an hour and it would get a signal 11 even if the server was just running on its 
own. I also do have the APIC stuff compiled into the kernel. I might try using the 
svga or fb driver because I bet its the tdfx driver because 4.0.3 used to work without 
a hitch.
