Can you try running the server with Option "SWcursor" in both of your r128 "Device " sections? This is needed for 2 radeon's setup, perhaps its the same for r128.
The problem is that the hardware cursor is trying to be moved or something early on in the server but the MMIO isn't mapped, something along those lines, I didn't spend much time debuggin it.
