XSLTForms 1.5 can now be downloaded at https://github.com/AlainCouthures/declarative4all/raw/master/public/direct/xsltforms.zip
The corresponding CSS stylesheet has been aligned to the one in the XForms Tutorial written by Steven Pemberton for Declarative Amsterdam: https://declarative.amsterdam/show?page=da-tutorial-xforms
