  1. New BodyVector written from scratch: supports body removal, intelligently 
allocates new ids at insertion. Is now default; a few pecent faster than 
BodyRedirectionVector which will be removed after a while.
  2. Remove BodyAssocVector
  3. Remove mass-spring for good
  4. Add body removal support to InsertionSortCollider
  5. Add empty body support to NewtonsDampedLaw, GravityEngine, 
OpenGLRenderingEngine and a few others
  6. Wrap body removal from python
  7. scripts/test/remove-body.py to demonstrate continuous dynamic body 
addition/removal.
