bm_pxe_ips is not used and should be dropped

Bug description:
The bm_pxe_ips table is not used any longer (IP allocation and
association is done by nova network or quantum). We should drop the
table and remove all its methods from baremetal/db/api.
Also, baremetal/volume_driver.py is the only place which selects from
the (always empty) bm_pxe_ips table. It should instead interrogate
nova network or quantum.
