"Create Volume from image" changes name of volume when selecting the
image to the name of the image

Bug description:
Steps to reproduce:
1. "Project" -> "Volumes" -> "Create Volume"
2. Fill in the volume name and size
3. Change the "Volume Source" to "Image"
4. Pick a value from "Use image as source"
Expected behaviour:
It should leave the user-specified values alone, so long as they're
not insane.
Actual behaviour:
It overwrites the user-specified values with the name and size of the
source image. This is terrible UX, and will cause unnecessary
confusion.
