Loadbalancer Healthmonitor status should be removed

Bug description:
After status has been added to PoolMonitorAssociation, the status of
HealthMonitor should be removed.
The reason for this is that HealthMonitor is a shared between balancers so no
single balancer should update it's status. Instead, status of association is
used to indicate status of HealthMonitor instance.
This makes HealthMonitor object a kind of template which is instantiated via
PoolMonitorAssociation
