LBaaS: add status of pool-monitor association to the pool return dict

Bug description:
'status' field was removed from the healthmonitor resource and instead added
to the pool-monitor association resource.
Currently there is no way for user to see the actual status of an association
- pool dict only enumerates the ids of associated monitors. So it would be
useful to include statuses there as well.
