Hung reboots periodic task should only act on it's own instances

Bug description:
Currently the hung reboots periodic tasks queries for *all* instances
in the database. It really should only querying for instances
belonging to it's own host.
This should use the `instance_get_all_by_host(context, self.host)`
pattern used elsewhere.
