need to sepcify region_name when multi region is available

Bug description:
in a multi region environment, all regions share the keystone auth service to
provide unified user credentials.
there can be multi endpoints for the same type to be used by different
region. in this case. when requesting a endpoint, region_name must be
specified or the follow error occurs

note that not only user who use a python-* client need to specify a
region_name when create a request but also we should add such
parameter in nova.conf.
