missing choice 'storage type' when creating a new volume during
instance creation

Bug description:
There should be a choice for the storage type when choosing 'boot from
image (create a new volume)' or 'boot from snapshot (creates a new
volume)' as instance boot source in the launch instance dialog in the
instances panel.
