optimise dhcp spawning performance

Bug description:
Some methods iterate over the same network ports list 3 times in
neutron/agent/linux/dhcp.py just to spawn a unique dnsmasq instance,
and potentially a network may have thousands of ports, so this adds a
lot of processing overhead.
For a mater of performance, some re-factoring in order to iterate only
once to generate all dnsmasq's configuration files (hosts, addn-
hosts, etc) is required.
