broken firewall table has broken policy/action values

Bug description:
As demo user navigate to:
Project - Network - Firewalls - Firewalls Rules. Create new rule by clicking
to Add rule. When the rule is created, there is missing empty cell in the table
with name of policy, that is part of it and it cause shift "Action" cell to
"Policy" column.
