RadioSelect is not displyed properly in modal or workflow

Bug description:
When RadioSelect is used as a widget of a form field within a modal or
workflow, the layout is off (see attached screenshot). It's caused by
the class form-field added in horizon/common/_form_fields.html sets
the full width for the input field.
