This sounds like a Horizon issue to triage. The server response should
have an image id in it, and some tools, like nova CLI, will show the
image name along with the image id, but I don't know about Horizon.

  Newton Openstack:

  On launching an instance using an image, once the image is created, no image 
name is populated against it the instance list or instance details. 

  
  Mitaka Openstack:
  -------------------
  On the Mitaka dashboard, Source panel on Launch instance window gives the 
option to create and attach a new volume of desired size along with the image 
used to create an instance. However, when I try to create an instance with an 
image and along with a new volume attached, the instance gets created 
successfully but does not show the image details.
