bq. I think it's better to leave out case 4. The right behavior on it is fuzzy,
and things are simpler if the results returned by QueuePlacementPolicy are only
a function of the configuration.
Removed queueMgr and using configuredQueues instead.

bq. Both NestedUserQueue and UserQueueBelow sound good.
Calling the rule 'NestedUserQueue'. Changed it at all places in the code.
I didn't add documentation in my last patch, I've changed FS doc in the
latest(v4) patch.
