In most (if not all) sections there is a 'refresh' function. It is called by
the paste routines and *most* times at initialisation. This initialises/updates
every single control - although occasionally I still find one that was missed.
At the same time, when created by make_window, almost every control has its
setting initialised again, and I found one place where a control was updated
three times in succession!
It seems to me that it would be simpler if, like paste, initialising always
calls refresh, and the controls are never initialised anywhere else.
