The shortcut "Add to Address Book" is under Tools in the Menu bar for
Entourage 2004. This adds the address for the sender.

To add an address in the body of the message. Double click and it opens
addressed to that person. You can then Control Click to select "add to
Address Book"

I agree, it should be a contextual menu item. Just did a quick Google search
and do see it is possible for a user to create a contextual menu item.
