So I ran zenmigrate and it seems to hang indefinitely:

INFO:zen.migrate:Installing AddEventMenuItem (2.2.0)

Any ideas?

You can force the step that create the maintenance window catalog.

zenmigrate --step=MaintenanceWindowCatalog
zenmigrate --step=TwoTwoIndexing
