Is your custom graph in a template you created? You'll need to create a ZenPack 
out of this template.

You can read how to do that, here:

Do you have COMMAND data sources that gather data from custom scripts? If so, 
backup the scripts you use.

Do you care about the data for that custom graph? If so, save the corresponding 
RRD's for your datapoints in $ZENHOME/perf/Devices/<device>

Install 2.2.4, install the ZenPack you created and bind the template to the 
devices you want the graph associated with.

If you want to see old data for devices, copy your backed up RRD's to 
$ZENHOME/perf/Devices/<device>
