I ran into this issue for a long time and was quite annoying.

In my case, it was all related to NTP. I'm running the zenoss server under vm( 
not using the appliance from zenoss), and was running ntpdate from cron to 
adjust the time every minute as running the daemon would fail for me for other 
issues related to the vm host machine and vmware.

 After cranking up ntpdate in cron from every minute to every 5 seconds, the 
problem dissapeared for good.

There are quite a number of tickets related to hearthbeats. You can also try  
clearing all hearthbeats that might be hanging on the database if you have 
changed the hostname by shutting down zenoss and clearing the hearthbeats 
manually from the sql table while zenoss is off-line.
