Just as an idea: use the python string modul for substitution.
Write in ReST:

.. figure:: path/to/image

   $title

   or

   ${caption}

and in your python code:

    theTitle = myPhoto.getTitle()
    theCaption = myPhoto.getCaption()
    theText = GetTheRestructuredTextFromMyData(...)
    theText = string.Template(theText).safe_substitute(theTitle, theCaption)
