hm, sequence_number is an integer. You can't add that to a string, hence
the error.

try something like (untested):

3:      <dtml-var "_['radiob%s' %_['sequence-number']]">

or

3:      <dtml-var "_['radiob'+_.str(_['sequence-number'])]">
