if you are applying for new card there it is asking for concession certificate by any Doctor or NGO but if you select for renewal upload the old Indian Railway E concession cardand mention the concession detail of any NGO mentioned in the old Indian Railway E concession card, I did not upload any concession certificate by NGO just selected the concession certificate coloum and entered National Federation of Blind which was entered in my old card.
