We got this a few times when the activation email was either blocked or altered by an incoming spam filters.  Check the user's inbox and make sure the activation email actually gets there when you are trying to activate. 

You might also see if the user installed BB Desktop manager on their workstation and chose "sync locally" instead of "sync with BES", although I'm not sure if that's still an option anymore.

Other accounts activate fine.
This account was activated on this BES server before.
Before setting the new activation code I saw that the account was listed twice in the BES web interface.
I deleted both. And, after 10 minutes, added the account once again.

I've also rebooted the BES server.

I can't think of anything else that might be causing this issue.
Any help appreciated.
