I've checked your document and I think there's a big error. You're
supposing that when you boot from Grub it checks your device.map with
your assignations and that it's not true.
It does check your bios settings... it receives configuration from bios.
So if your bios does not detect your usb device as a bios device or a
bootable device it won't appear it on grub and thus you won't be able to
boot it from grub.
I think you should change:
This procedure can enable booting from a USB device on machines that do
not support USB boot devices in their BIOS.
to:
This procedure can enable booting from a USB device on machines that do
not support booting from USB boot devices in their BIOS but the BIOS
sees them as hard disks.
