We only use GUIDs in situations where we are sharing data. If you have worries / troubles about moving from dev to staging to live, then you'll need them everywhere. Basically, one is the primary key (The Integer), one is an "alternate" key (The GUID). I don't think their is a way to specify an alternate Primary key in SQL Server. We haven't had any problems (that were related to the "dual keyed" approach).
When DB changes happen, we use a tool called APEX SQL Dif to help sync the table structures between dev and live. <http://www.apexsql.com/> I believe this tool will sync up table data also.

Jeffry-
How do you manage having both identity integers and GUIDs? Im leaning toward this approach, but have concerns over maintenance etc. Do you use the integer as the PK/FK and just have the GUID sitting there for when you need to communicate between systems?

Probably this one: http://www.cflib.org/udf.cfm?ID=512
Although this one may be suit the purposes to: http://www.cflib.org/udf.cfm?ID=54
I'll add that I never use GUID over "identity ints". I haven't worked in environments where replication has been an issue either. I can see how there would be benefit in avoiding replication conflicts.
I will use GUID in situations where I'm integrating with other sources. This is usually in addition to an internal identity, not instead of.

I'll add that there is a UDF that "fixes" CF's GUID to resemble SQL's GUID. I thought it was on CFLib, but I couldn't find it. Google didn't find it either. Hopefully, someone else can remember where it is.
