If you look at the picture at http://www.tavia.com/cat12.html, you'll
see two threaded bars - one for each thread size.  You'll also see the
large faced nut and "washer."  Thread the correct size end into the
crank snout and use the nut to draw the balancer onto the crank.  The
"washer" spreads the load out so it doesn't strip the threads in the
crank.
