As an additional data point, it appears some apps honor the setting and others don't. The setting itself does get recorded somewhere: if you 'xset b 0' and then 'xset q', the change in volume is shown in the output from 'xset q'.

Among terminals I've tested:
- xterm beeps even when bell is set off
- rxvt-unicode uses the setting as expected
- aterm uses the setting as expected

GTK apps also ignore the setting: if you backspace in an empty textbox in a GTK app like Pidgin, you get a beep even when the bell is set off.

I assume rxvt-unicode and aterm must be using a different codepath for their bells that still works?
