If i had to guess i would say it is because of this ( most likely i am
wrong since i looked at it for like 1min )
# grep -R refresh *
program/js/app.js: add_url += '&_refresh=1';
# grep -R app.js *
program/include/main.inc: $OUTPUT->include_script('program/js/app.js');
# grep -R main.inc *
index.php:require_once('include/main.inc');
I would say since main.inc is required by index and app.js is included
via main.inc that is where it is getting the refresh
