Duplicated replacement orders are created when "replacement" returns are
accepted.
How to recreate:
1) create an order and quick ship it
2) go to the order detail screen and click on "create return"
3) select return type id = "Replacement" and check the checkbox for the order
items you want to return; submit the form
4) go to the return header screen and change the status to "approved"
5) go to the order detail screen: you will see to replacement order links
associated to the returned order items.
I suspect that the same may happen for other types of returns.
