I did not see any option to download "setup.exe". The general download
button got me a file called "winsdk_web.exe", which I have run with
default settings.

Step: Optional: Get dbghelp.dll (for using the --enable-dbgutil configure option)

This step contains the first mention of MS Visual Studio, but seems to assume it is already installed. I do have a version of it installed, but not the file dbghelp.dll.
