Add a new tag, <display:header>, conceptually parallel to <display:footer>. If
present, it specifies additional HTML (presumably TRs) to be rendered above the
generated header row, but still in THEAD.
Would address the issue in DISPL-242 and DISPL-151, but is not the solution
proposed in either of those, so I'm opening this new issue.
Attached tarball includes an implementation (one new file, plus a diff against
displaytagCVS), a unit test (three new files) and an example (diff against
displaytag-examples CVS).
