Based on your suggestion, I took a second look at the meta data field registry and input-forms.xml. Looking at input-forms.xml, I edited this file by removing the field for the dc-element type. This edit fixed the problem and the input forms worked again. Next I looked at the Meta Data Field Registry (under the administration section). I found for some reason that there was no _type_ element defined in the registry. So, the actual problem was that at some point the dc-element _type_ was removed from my meta data field registry. After adding it back removed the error and fixed my problem. Thanks for your suggestions.
