You opened a dired buffer and yes, it's gone when the mouse cursor  
crosses over the text cursor. It returns, as a hollow rectangle, when  
you select a different Aqua client. This hollow rectangle becomes  
completely grey when Emacs.app is reselected.

The hollow rectangle is the text cursor's shape in an inactive window  
inside that frame. This inactive cursor also vanishes when the mouse  
cursor crosses over it in the inactive window.
