I corrected my mistake and now have /usr/local/etc/asterisk/extensions.conf

My bad, I thought it was only for hardware which I don't have. I installed
it anyway. Remember I have NO experience with asterisk.

There are no firewalls on this server. netstat shows the server is listening
on port 4569
