I can’t speak to you other questions. But, regarding FDI-SETUP, at present, all packages (except V8POWER and the WELCOME message package) are pulled from Mateusz’ ALL_CD.ISO. The Floppy image pulls them from that CD. The USB pulls them from whichever version of that CD I am using. (Currently the Jan 5, 2016 version) I know this doesn’t really answer you question.
