I haven't seen that on my own server, but I got booted from another server for this reason after downloading a custom map.
As I recall, this was shortly following the Item Server outage yesterday, so it may be related to that? Granted, I had restarted TF2 (to get my loadout back) and reconnected to the server prior to the map change. Incidentally, I've noticed an increasing number of Item Server outages that cause loadout changes to stop working until the client completely exits TF2 and rejoins. It doesn't even show the usual "Can't connect to item server" thing, you just can't change loadouts.
