Look at the right column ("Downloads") under Resources.
Download Chapter 6 and read section 6.3.2.
There's an example where a PDF is created without headers/footers (first 
pass).
You can skip that first pass, as you already have a PDF file.
You need the second pass: this is where a header/footer is added to each 
page.
