As far as I know this was already answered. You are rotating content (PDF syntax in a page stream or XObject) instead of rotating the annotation. Rotate the annotation as explained before. Whether or not you're using iText is irrelevant: annotations (e.g. RichMedia annotations with a swf file) aren't part of the content. They are added "on top of" the content.
