I work on a project for Actuate, the project lead of Eclipse/BIRT. They asked me if I could make them a Tagged PDF example with an alt tag for an image. You can find this example here: http://itext.ugent.be/itext-in-action/chapter.php?chapter=11#bonus (Search for ReadOutLoud). For the moment the approach is very low-level, but in iText 2.0 I introduced the concept of MarkedContent. I intend to make it easier to create Tagged PDF using those objects in the future, but I can't give you a time frame.
