Yes.  You'll have to use the low-level PDF object classes, PdfDictionary,
PdfArray, PdfString, etc.

And you'll need to constantly check the PDF Reference as you go, so you'll know
what it is you're supposed to be reading or writing.

I also recommend RUPS, so you can view your input or output at that same low
level.  Without that, you're shooting in the dark till Acrobat magically says
"you got it" one day.

For most of the objects you're working with, a plain old text editor would be
okay for checking things, but RUPS (or a similar program, like WindJack's PDF
CanOpener: a commercial Acrobat plugin, free trial available IIRC, I'm not
affiliated with them in any way) is much more convenient.
