I was able to make this work. I had placed additional HTML on the page after the PDF document was opened and closed. I made the output of the additional HTML (input fields, title, instructions, etc) conditional upon the existence of an error and the initialization of the page. As it turns out the Adobe 9 reader handles the additional garbage better then version 7.
