Thanks Michael, 
I didnt think of adding text to the appearance.  It works really great for
my first signing without annotation modified warning. But here comes another
problem discovered... when i made the second signing and annotation by the
same code, the annotation modified warning appear again for the annotation
added after first digital sign.
