Unfortunately as soon as I tried to reboot with debug info on the problem
disappeared and now I cannot reproduce it anymore. It remains elusive for me.

I have the same problem. It looks like we have the same rev card as well. I've tried several firmware and driver versions as well as various module options to no avail. Here's my dmesg output:
