I fear part of this bug might be related to another one, as the sites we used to download cover from have all but one changed their conditions on cover fetching, currently covers can only be downloaded from last.fm (bug #328230). As for why Amarok doesn't show all embedded covers: can you please check if there is a difference in file type between those who show and those that don't?
