I don't really know if it applies to 2.6.21 since I haven't been able to
reproduce it with that kernel. I suspect it does but is harder to
trigger due to slightly different timing possibly caused by us not
freezing kernel threads for suspend right now.

Hence, since nobody seems to have discovered this before, I'd say it is
post-2.6.21.

No, it seems to be scheduled a few times over a few files with varying
timeouts. I could try to put in a few printks but most likely I won't be
able to actually see them during suspend (no serial, netconsole could be
suspended, ...)
